The California Chronic Care Coalition (CCCC), together with Axis Advocates, invites you to join this meeting focused on improving healthcare in Southern California. Join us to share the healthcare access gaps affecting your members, learn from others, and work together on solutions to reduce those barriers. We’ll be talking about key health issues like:

• Obesity
• COPD
• Environmental impacts on health
• Health care denials
• Pain management: Special focus — We’ll discuss new advances and better ways to manage pain for people living with sickle cell disease
